Modelling sociodemographic determinants of health-related quality of life (HRQoL) loss among older adults in Spain using discrete beta regression

Objectives: This study aims to identify the main factors associated with health-related quality of life (HRQoL) loss among older adults in Catalonia (Spain), and to evaluate alternative regression frameworks for health utility outcomes.
Methods: Using data from the Catalan Health Survey (ESCA) for 2024, the study focuses on individuals aged 60 and over. HRQoL loss was analysed using discretised beta regression (DBR) and compared with zero-one inflated beta models.
Results: DBR showed higher predictive accuracy. Greater HRQoL loss was associated with economic hardship, weak social support, sex, obesity, disability and limited mobility. Ageing appears associated with greater loss, but not after adjusting for disability, indicating that deterioration is driven by functional decline rather than age itself.
Discussion: DBR provides a flexible and theoretically grounded alternative to the conventional regression commonly applied in HRQoL analysis
